The medical process is not only about treating diseases.

[Approaching the most beautiful doctor]

The medical process is not only about treating diseases.

Shao Zhimin, Director of Breast Surgery and Director of Major Surgery, Fudan University Cancer Hospital

I have been engaged in basic and clinical research of breast cancer for nearly 40 years, and I have the honor to participate in and witness the rapid development of breast cancer diagnosis and treatment. For decades, I have worked hard to heal the physical and mental pain of breast cancer patients in the clinical front line and tried to become a "ferryman" of their lives.

As the largest breast cancer diagnosis and treatment center in China, the Breast Surgery Department of Fudan University Cancer Hospital attracts patients from all over the country every day. Some types of breast cancer, such as triple negative breast cancer, lack therapeutic targets, are difficult to treat and have low cure rate.

"As long as 1% of breast cancer patients do not reach the level expected by doctors and patients, doctors must make 100% efforts to tackle the problem." Since I became a doctor, I think so, and I also practice it with my team.

I found that some patients with triple-negative breast cancer can successfully "cross" the "hurdle" of 5 years; Others relapse and metastasize in a short time. This has inspired me to think scientifically: Can triple negative breast cancer be subdivided, classified, treated accurately and improved in curative effect?

Since 2009, I have led my team for nearly 10 years, and gradually explored the way forward on the road of precise treatment of triple-negative breast cancer. I have gone through four stages: finding the optimal chemotherapy scheme, revealing the molecular essence by multi-omics, precise treatment based on molecular typing, and optimizing typing to expand the connotation, thus changing the history that triple-negative breast cancer has no effective treatment target in the past. These research results were applied and popularized in clinic for the first time.

The whole study was extremely difficult, during which I deeply realized the anguish when I studied "hitting a wall" and the joy after the breakthrough. The effort of "grinding a sword in ten years" is not just to win several awards, send several articles and apply for several projects, but to benefit hundreds of thousands of patients. This is my initial intention to engage in research and the driving force for me to keep moving forward.

Behind every patient is a family. Behind entrusting life is the patient’s trust in the doctor. Every day when I walk into the ward and see the patients, my heart will be stable; Every outpatient day, my words, a smile and an encouragement are also the "reassurance" of patients.

I got into the habit of getting up at 4: 00 a.m. every day, dealing with the mail I received at night and starting to arrange the clinical, research and management work that day; Arrive at the hospital at 6 o’clock in the morning to make rounds and "talk" with hospitalized patients. The patients after the operation are more or less anxious and afraid. When I come to them, their nervous mood and their families will gradually relax and pat their shoulders: "The operation was very successful, and the follow-up treatment will be arranged for you"; "Your recovery from illness will not affect your future marriage and childbirth, so don’t have a burden on yourself" …

I found that sometimes a shallow smile can make patients feel like spring breeze. I have always advocated that the medical process should not only focus on the word "treating diseases", but more importantly, it should heal the injured hearts of breast cancer patients and help them gain confidence and courage to return to a normal life track.

In the outpatient clinic, my expert seats are almost empty, and I keep walking between the physical examination and the consultation desk to improve the efficiency of seeing a doctor. I want to "lock in" the core information in the treatment report at the first time and give advice on diagnosis and treatment accurately and decisively. My words "You are all right, remember to follow up" often make patients’ locked brows stretch. When I meet some depressed patients, I will also share our latest 5-year survival rate data with them, which is more comforting and encouraging: "Trust us, you must cooperate with us, and your illness will be controlled if you are in a good mood and receive standardized treatment …" Every time I talk about these data of treatment benefits, a "light" will flash in patients’ eyes, which is a true expression of the future vision. Even for patients with refractory breast cancer with recurrence and metastasis, I also give them confidence and transmit positive energy, so that patients can fully feel that "other patients can get out of the haze and live a wonderful life, and I can also survive with tumors for a long time …"

When I meet some breast cancer patients who are in urgent need of psychological intervention, I will recommend them to the department of psychological medicine of our hospital at the first time, so that they can do a good job of positive psychological counseling and support while treating breast diseases, and establish a positive and optimistic attitude by means of "peer support" to cooperate with the treatment and achieve the expected therapeutic effect.

The development of our breast cancer discipline is a process of "ten years of trees and a hundred years of people". My tutor, Professor Shen Zhenzhou, has taught me everything from surgical skills to scientific research. Now, as a leader of the discipline, I also put more energy into cultivating more high-level talents, so that they can take the baton from me in the future and get a greater breakthrough in curative effect.

In the past, I worked in American scientific research institutions for more than 10 years. Now I can proudly say that our breast cancer clinical research team has been comparable to the developed countries in terms of laboratory infrastructure construction, research ability and level, gathering of scientific and technological talents, and output of high-quality research results. The level of breast cancer treatment in our hospital has achieved a great leap forward, but it needs greater innovation and efforts to move towards a higher level. shine on you is better than blue, and the future belongs to every young person with great love, innovation and dedication.
